Simplify the interface for most cowboy_req functions. They all return
a single value except the four body reading functions. The reply functions
now only return a Req value.
Access functions do not return a Req anymore.
Functions that used to cache results do not have a cache anymore.
The interface for accessing query string and cookies has therefore
been changed.
There are now three query string functions: qs/1 provides access
to the raw query string value; parse_qs/1 returns the query string
as a list of key/values; match_qs/2 returns a map containing the
values requested in the second argument, after applying constraints
and default value.
Similarly, there are two cookie functions: parse_cookies/1 and
match_cookies/2. More match functions will be added in future commits.
None of the functions return an error tuple anymore. It either works
or crashes. Cowboy will attempt to provide an appropriate status code
in the response of crashed handlers.
As a result, the content decode function has its return value changed
to a simple binary, and the body reading functions only return on success.

The code for parsing has also been rewritten to be more efficient
and to be able to handle cookie values with space inside them properly.

Previously cowboy_http:te_chunked/2 would enter an incorrect state if
it tried to parse an incomplete chunk when the length was known from the
partial chunk.
Previosuly cowboy_http:te_chunked/2 expected the trailing "\r\n" to
always be present if chunk body was present in the buffer. This is not
guaranteed and so this commit accommodates that situation.

We know this specific parameter is case insensitive so we
automatically lowercase it to make things simpler to the
developer.

Now instead of {1, 1} we have 'HTTP/1.1', and instead of {1, 0}
we have 'HTTP/1.0'. This is more efficient, easier to read in
crash logs, and clearer in the code.

and will prevent errors due to failed expectations.

Defaults to a maximum of 1000000 bytes.
Also standardize the te_identity and te_chunked decoding functions.

The code may not look as beautiful as it was before. But it really
is, for parsing code. The parsing section of the file may be skipped
if your eyes start to burn.

Header names are now binaries. Since header names are case insensitive
they are all converted to lowercase. For example: <<"content-length">>.
The max_line_length option was removed. Three new options have been
added instead:
* max_request_line_length (defaults to 4096)
* max_header_name_length (defaults to 64)
* max_header_value_length (defaults to 4096)

First step in making all methods and header names binaries to
get rid of many inconsistencies caused by decode_packet/3.
Methods are all binary now. Note that since they are case
sensitive, the usual methods become <<"GET">>, <<"POST">> and so on.

Use a proper HTTP client to run all tests. This client is currently
undocumented and should not be used.
Includes a few fixes:
* Fix a bug in the max_keepalive test
* Fix a bug with max_keepalive handling
* Fix a bug in stream_body/1 where data was lost under some conditions
The tests now run quite faster than before.
All the tests now run twice: once for TCP, once for SSL.

Certain user agents send invalid Accept-Charset headers, like the
following: "ISO-8859-1;utf-8;q=0.7,*;q=0.7"
The user agent with which this behavior was observed presented itself
with the User-Agent string: "Mozilla/5.0 (Windows; U; Windows NT 5.1;
en-US; rv:1.9) Gecko/2008052906 Firefox/3.0" Although this doesn't
appear to be correct. The request might have been mangled by a
transparent proxy.

Introduces 3 low level functions and updates the existing higher
levels functions. The new primitives are has_body/1, body_length/1
and stream_body/1. In addition to that, a helper function
init_stream/4 has been added.
Streaming a body implies to decode the Transfer-Encoding and
Content-Encoding used for the body. By default, Cowboy will try
to figure out what was used and decode them properly. You can
override this if you want to disable this behavior or simply
support more encodings by calling the init_stream/4 function
before you start streaming the body.
