Revamp non-EVP aes_ctr_stream_encrypt()
* Moved AES_set_encrypt_key() call until after all arguments are parsed.
* Added bounds check before casting.
* Added error handling for all OpenSSL calls.
* Noted when unusual OpenSSL API call returns 0 on success.
