Fix encoding of semi-constrained, extensible INTEGERs
Given:
Semi ::= INTEGER (Lb..MAX, ...)
where Lb is an arbitrary integer, attempting to encode an
integer less than Lb would cause the encoder to enter an
infinite loop.
