This commit improves crash dumps in several ways:
* Suspends schedulers to get a current snapshot
* Dumps information about scheduler
* Dumps stack trace of current running process
(including Garbing processes)

Change so that we print that the we generating a crash dump
before we actually generate it. So that if a huge crashdump is
being dumped, the user knows that something is going on.

A process requesting a system task to be executed in the context of
another process will be notified by a message when the task has
executed. This message will be on the form:
{RequestType, RequestId, Pid, Result}.
A process requesting a system task to be executed can set priority
on the system task. The requester typically set the same priority
on the task as its own process priority, and by this avoiding
priority inversion. A request for execution of a system task is
made by calling the statically linked in NIF
erts_internal:request_system_task(Pid, Prio, Request). This is an
undocumented ERTS internal function that should remain so. It
should *only* be called from BIF implementations.
Currently defined system tasks are:
* garbage_collect
* check_process_code
Further system tasks can and will be implemented in the future.
The erlang:garbage_collect/[1,2] and erlang:check_process_code/[2,3]
BIFs are now implemented using system tasks. Both the
'garbage_collect' and the 'check_process_code' operations perform
or may perform garbage_collections. By doing these via the
system task functionality all garbage collect operations in the
system will be performed solely in the context of the process
being garbage collected. This makes it possible to later implement
functionality for disabling garbage collection of a process over
context switches.
Newly introduced BIFs:
* erlang:garbage_collect/2 - The new second argument is an option
list. Introduced option:
* {async, RequestId} - making it possible for users to issue
asynchronous garbage collect requests.
* erlang:check_process_code/3 - The new third argument is an
option list. Introduced options:
* {async, RequestId} - making it possible for users to issue
asynchronous check process code requests.
* {allow_gc, boolean()} - making it possible to issue requests
that aren't allowed to garbage collect (operation will abort
if gc should be needed).
These options have been introduced as a preparation for
parallelization of check_process_code operations when the
code_server is about to purge a module.

When dumping the heaps and stacks in the erl_crash.dump file, only
the lower 32 bits of pointers would be printed. Ensure that all bits
are printed.
While at it, make sure that printing sub binaries with a debug compiled
run-time system will work.

Crashdumps initiated by out-of-memory on spawn could cause the beam
to segfault during crashdump writing due to invalid pointers.
The pointers are invalid since the process creation never finished.
This commit remedies this problem by removing the process from
crashdump printout.

This changes the behaviour back to the R15B02 default considering
if a beam was running with no heart.
The motivation is that we want a fast heart to fast restart erlang if
it crashes but to be able to get non-truncated erl_crash.dumps via
setting ERL_CRASH_DUMP_SECONDS if we so desire.

Not setting ERL_CRASH_DUMP_SECONDS will now terminate beam
immediately on a crash without writing a crash dump file.
Setting ERL_CRASH_DUMP_SECONDS to 0 will also terminate beam
immediately on a crash without writing a crash dump file, i.e. same as not
setting ERL_CRASH_DUMP_SECONDS environment variable.
Setting ERL_CRASH_DUMP_SECONDS to a negative value will let the beam wait
indefinitely on the crash dump file being written.
Setting ERL_CRASH_DUMP_SECONDS to a positive value will let the beam wait
that many seconds on the crash dump file being written.
A positive value will set both an alarm in beam AND a heart timeout for restart
if heart is running.
This is due to the change of 'heart' behavior when 'heart' is
listening for a crash.

Putenv and getenv needs to convert to the proper environment
strings in Unicode depending on platform and user settings for filename
encoding. Also erlexec needs to pass environment strings in an appropriate
way for kernel to pick up. All environment strings on the command
line, as well as home directory, is now passed in UTF8 on windows
and in whatever encoding you have on Unix, kernel tries to convert all
parameters and environments from UTF8 before making strings.

The ERTS internal system block functionality has been replaced by
new functionality for blocking the system. The old system block
functionality had contention issues and complexity issues. The
new functionality piggy-backs on thread progress tracking functionality
needed by newly introduced lock-free synchronization in the runtime
system. When the functionality for blocking the system isn't used
there is more or less no overhead at all. This since the functionality
for tracking thread progress is there and needed anyway.

As a preparation for changing the calling convention for
BIFs, make sure that all BIFs use the macros. Also, eliminate
all calls from one BIF to another, since that also breaks
the calling convention abstraction.

All uses of the old deprecated atomic API in the runtime system
have been replaced with the use of the new atomic API. In a lot of
places this change imply a relaxation of memory barriers used.

Existing %bp to print pointer size integers does not work in halfword
emulator to print Eterm size integers.

For cleanliness, use BeamInstr instead of the UWord
data type to any machine-sized words that are used
for BEAM instructions. Only use UWord for untyped
words in general.

Store Erlang terms in 32-bit entities on the heap, expanding the
pointers to 64-bit when needed. This works because all terms are stored
on addresses in the 32-bit address range (the 32 most significant bits
of pointers to term data are always 0).
Introduce a new datatype called UWord (along with its companion SWord),
which is an integer having the exact same size as the machine word
(a void *), but might be larger than Eterm/Uint.
Store code as machine words, as the instructions are pointers to
executable code which might reside outside the 32-bit address range.
Continuation pointers are stored on the 32-bit stack and hence must
point to addresses in the low range, which means that loaded beam code
much be placed in the low 32-bit address range (but, as said earlier,
the instructions themselves are full words).
No Erlang term data can be stored on C stacks (enforced by an
earlier commit).
This version gives a prompt, but test cases still fail (and dump core).
The loader (and emulator loop) has instruction packing disabled.
The main issues has been in rewriting loader and actual virtual
machine. Subsystems (like distribution) does not work yet.

NIF function prototypes in order to allow more than 3 function
arguments. Also an incompatible change in the return value of
erlang:load_nif/2. Added support for references, floats and term
