Instead of initiating running of the app tests for asn1 from
asn1_SUITE, put the tests in asn1_app_SUITE and let it take care
of itself.
While we are it, eliminate 'export_all' in the new module and
use lists:keyfind/3 instead of lists:keysearch/3.

The old code for testing DEFAULT values is messy, inefficient, and
causes dialyzer warnings.

Always pre-encode the values for the enumeration. Clean up the
code and let the per and uper back-ends share the code.

All table access is now performed in a separate module. This will allow
changes to how ETS is handled by changing only this module. Note that
the module exports a very ETS-like interface for now which would have to
be maintained even if the data format would change (to a hash map for
example).

Two modules were clearly copied and adapted from the megaco
test suite. Change the name of the application being tested
in a comment to "asn1" and also remove out-commented code
that refers to "megaco".
