* rickard/make-fixes-20/OTP-15551:
Fix install phase in build system
- Install of (mainly) documentation caused rebuild and
modification of the source tree even when the source
previously had been built. Also otp_patch_apply
modified the source tree when updating documentation.
This messed up the installation if installation was
performed by another user than the user that originally
built the system which not is an uncommon scenario.
- Some documentation was installed by copying files
instead of installing the files which caused faulty
access rights on files.
- The documentation was not properly updated when
applying a patch using otp_patch_apply.

Prior to this change, calls inside binaries were
treated as top level which would cause the `Fun(Arg)`
call inside `<<Fun(Arg)/binary>>` to return an internal
dbg_ieval tuple and ultimately error with badarg.

The bug was introduced in commit 7eaaf5.

The efile driver will soon be reimplemented as a BIF.
Instead of opening a port based on efile, use hd(erlang:ports()). It
is a reasonable safe assumption that the runtime will continue to use
use at least some ports.

This commit also adds a check to see that all files that
are part of an xi:include also have part of XML_FILES
and vice versa. It also fixes any applications where this
was not true.

The extended parser is used by the debugger for handling pids, ports,
references, and external funs in the evaluator area and in the
bindings area.
Notice that pids, ports, references, and external funs that cannot be
created in the currently running are ignored (as before).

The evaluated string looked like "V=<<1>>", which cannot be scanned
correctly. A space after "=" fixes the bug.
