After DPR, the intention is that outgoing answers are sent and outgoing
requests are discarded. This was the case when the DPR was outgoing, but
if it was incoming then a subsequent outgoing request resulted in a
function_clause, bringing down the diameter_peer_fsm gen_server
associated with the transport connection in question.
With diameter_tcp/sctp, the failure can result in the connection being
reset before the peer closes it in response to DPA, but is otherwise
harmless since DPR means that the connection is already on its way down.

Referencing a matched-out variable in a size expression makes it
impossible to calculate the size of the result based on the size of
the matched binary. The compiler would still generate code to do
this however, which would crash since the variable isn't defined
at the size calculation.

If the report callback is a fun of arity 2, then the second argument
is a map with the keys 'encoding', 'depth' and 'chars_limit', and the
fun must return a string which is size limited according to the given
depth and chars_limit.
If the report callback is a fun of arity 1, then it must return a
tuple containing a format string and a list of arguments. The
formatter will produce the string, and limit it's size.

longs, longlongs and wchar were too small on 64-bit
which could lead to potential buffer overflow at encoding.
__OE_DOUBLESZ__ was too big, probably due to old text format.
