This checks if the module is already loaded using erlang:module_loaded before calling the code server. This should improve performance of the call significantly since the case where module is already loaded is the common one. The change shouldn't cause any problems. It's worth noting that code:ensure_modules_loaded already does a similar check.

Prior to this change, calls inside binaries were treated as top level which would cause the `Fun(Arg)` call inside `<<Fun(Arg)/binary>>` to return an internal dbg_ieval tuple and ultimately error with badarg.

Timestamps have been monotonic time since OTP-17.5. See commit e6d19a18. The examples should really be replaced by more detailed doc of return values.
