Parameterized modules are no longer supported, so module() can only be
an atom().

The recently added module erl_anno can no longer handle
negative line numbers.

This ticket is about records in Erlang code, and when to check the
fields against the (optional) types given when defining records.
Dialyzer operates on the Erlang Core format, where there are no trace
of records. The fix implemented is a Real Hack:
Given the new option 'dialyzer' erl_expand_records marks the line
number of records in a way that is undone by v3_core, which in turn
inserts annotations that can be recognized by Dialyzer.

The code related to the introduction of unicode_string() and
unicode_char() has been removed. The types char() and string() have
been extended to include Unicode characters.
In fact char() was changed some time ago; this commit is about
cleaning up the documentation and introduce better names for some
functions.

Dialyzer no longer outputs warnings for unused anonymous functions
("funs"). Warnings are still output for unused functions.
Funs in functions with -nowarn_unused_function attributes generated
warnings for unused functions. Unused list comprehensions also
generated warnings for unused funs.

Dialyzer was not reporting unused functions with 0 arity. This was not
a real issue, until we found out that there could be cases where this
could lead to false warnings. This was the case in "no_local_return.erl".

Enable warnings by default, add two options for suppressing them,
fix warning formatting and update testsuites.

Dialyzer does not normally emit warnings for functions that
implement non-terminating server loops. This detection failed
when some of the elements in an SCC terminated normally (being
for example list comprehensions or other generic anonymous
functions that were included in the SCC). This patch fixes that.
